• Name of patient

• Date of birth (required for insurance verification when we contact them to inquire about your acupuncture benefits)

• Photo of the front and back of your insurance card

VETERANS: Please let your VA Primary Care Physician know you are interested in a referral for acupuncture and provide our below contact information. Tri-West covers the cost of your acupuncture treatments. Please be patient as they process the pre-authorization and then fax to us. Feel free to reach out to us so we can assist in expediting the process.
